Before dispensing the prescription

Double-check the accuracy of the prescription.

Provide undivided attention when filling the prescription.

1. Check the patient information (e.g., name, address, date of birth, telephone number).

2. Check the patient profile (e.g., allergies, medical conditions, other drugs, including over-the-counter medications).

3. Check the drug (e.g., correct drug name, correct spelling, appropriate drug for the patient's condition), and verify that there are no known drug interactions. Always verify the name of the drug. Beware of drug names that look alike (see table).

4. Check the dosage, including the drug strength; the dosage form (e.g., capsule, liquid, modified release); the individual dose; the total daily dose; the duration of treatment; and the units (e.g., mg, ml, tsp, tbsp).

5. Check the label. Compare the drug dispensed with the prescription. Verify the National Drug Code (NDC) number. Ensure that the information is accurate, that the patient directions are accurate and easily understood, and that the auxiliary labels are appropriate.

6. Provide patient counseling. Make sure that the patient fully understands the drug treatment as well as any precautions.

Drug labeling

Proper labeling is one of the most important aspects of dispensing a prescription. The label must comply with state and federal regulations and should correctly and clearly convey all necessary information regarding dosage, mode of administration, and proper storage of the product. The quality of the labeling is extremely important to the patient's perception of the quality of the product and may have profound implications for his or her safe use of the medication and compliance with the prescribed regimen.

Required label information

Most pharmacies have the name, address, and phone number of the pharmacy pre-printed on their prescription labels. In addition, federal and state regulations require that the following information appear on the label.

- prescription (serial) number;

- date of initial dispensing;

- patient's name;

- directions for use;

- name and strength of the drug product (or active ingredient(s) in a compounded prescription);

- prescriber's name;

- name of dispensing pharmacist;

- expiration date.

Labels for controlled substances in schedules II, III, and IV must also contain the statement «Caution: Federal law prohibits the transfer of this drug to any person other than the patient for whom it was prescribed» in the form of an auxiliary label.
