12.07.2012. During the day ( I ) have been working in the treatment room of therapeutics department. Was helping a nurse prepare 5 drip sets for intravenous infusion, helped (15 I.Vs) and delivered independently 5 I.V. injections. Delivered 5 subcutaneous, 12 intramuscular injections. Made ECG recording of a critically ill patients (1). Carried out blood taking from 10 patients (vein) for biochemical tests. Delivered intra/m and intravenous injections to:

1. A patient with chronic cholecystitis attack:

Sol. Drotaverini hydrochloridi 2%-2,0 intra/muscular;

2. A patient with hypertensive crisis:

Sol. Pentamini 5%-1,0 i/m, single dose;

3.  A patient with chronic bronchitis attack:

Sol. Euphilini 2,4%-10,0 i/v.

List of practical skills for internal diseases

 

A student should know how to:

1. Examine a patient: questioning, examination, palpation, percussion, auscultation of organs and systems. To determine the severity of the patient's condition.

2. Formulate the diagnosis according to accepted classifications of diseases (primary disease, complications, comorbidities), substantiate it, make a plan for further tests and treatment.

3. Assess the values of complete blood cell count and findings of biochemical blood test; know the measurement standards and diagnostic value of lapses during various diseases: the content of total protein and its fractions, activity of AST and ALT, content of urea and creatinine, index activity of inflammatory process (CRP, fibrinogen, etc.) and streptococcal infection (AS(L)O, AST), lipid spectrum, pigment and purine metabolism.

4. Assess the main indixes of thromboelastogram and coagulogram, electrolyte and acid-base metabolism.

5. Assess and know the practical significance of common urine test, Zimnitskiy test, Nechiporenko test, Rehberg test, clinical and bacteriological analysis of sputum.

6. To perform simple laboratory tests: ESR, L, Нb, clotting time, blood glucose, common urine test, urine test for albumine, sugar, acetone bodies, benzidine test, pleural and ascitic fluid test, Zimnitskiy’s test, blood typing and RH factor.

7. Record an ECG, perform Holter ECG monitoring, bicycle ergometry, to carry out the function tests: hyperventilation, orthostatic, medication. To investigate the function of external respiration. To interpret the results.

8. Use the data of laboratory, instrumental, ultrasound, x-ray examination for making a diagnosis.

9. Know the difference between the norm and pathology on x-rays pictures of lungs, heart, abdominal organs, biliary tract and kidneys, spine, other bones and joints.

10. Choose rehabilitation measures in case of myocardial infarction (heart attack).

11. Diagnose and render first and professional medical aid in case of emergencies such as: sudden cardiac arrest, status anginosus, paroxysmal tachycardia, Morgagni-Adams-Stokes syndrome, hypertensive crises, acute cardiovascular failure, left ventricular failure (cardiac asthma and pulmonary edema), pneumothorax, pulmonary embolism, bronchial asthma attack and asthmatic status, liver and renal colic, diabetic, hepatic and uremic coma, anaphylactic shock and other allergic reactions, household and occupational poisoning.

12. Draw up a plan of patient’s examination during:

 - chest pains (thoracalgia);

 - arterial hypertension syndrome;

     - cardiomegaly and heart murmur;

   - heart rhythm disorder;

 - heart failure;

 - edema syndrome;

 - articular syndrome;

   - prolonged shortness of breath;

   - pains in the abdomen;

   - protracted dyspepsia and frequent constipation;

   - altered urinary sediment;

   - generalised enlargement of lymph nodes;

   - prolonged fever.

14. Suspect and organise competent advice during the course of:

- infectious diseases, including especially dangerous infections:

     - tuberculosis;

    - mental and neurological diseases;

- ENT-diseases;

- surgical and gynecological disorders;

- tumours.

15. Have a chat with patients and their relatives, to lecture on preventive health topic.
